Inverclyde community diabetes service 

Who are we ?

 The community Diabetes Specialist team is a small team that consists of 2 disbetes specialist Nurses and 1 diabetes specialist dietitian . 

Team memebers :

Doreen Kennedy : diabetes spcialist nurse / Team lead                           ( Part time )

Linsey Mawer  : diabetes specialist dietitan / Team lead                           ( Full time )

Heather McGhee :  diabetes speicat nurse                                                 ( Full time )

 

What do we do ?

The team provides a specialist diabetes management, treatment and education service for people with diabetes living in Inverclyde. This includes initiation of diabetes treatments, provision of specialist diabetes clinical/ dietetic support and advice. The team also provide diabetes group education and one to one education sessions for people newly diagnosed with Type 2 diabetes.

 

How does it work ?

Your GP, Consultant, Health Care Professional or Social Worker may have referred you to the specialist diabetes service. We will assess your diabetes needs at the ICIL community diabetes clinic or in your own home / care home and agree an individual diabetes care/ treatment plan in partnership with you. You may then be offered a review appointment or a pre booked telephone / video appointment for specialist treatment support and advice following your initial consultation.

You will not be required to attend the service long term. On average you will be invited to attend 2-3 appointments depending on your individual needs.  Due to demands we may have a waiting list so please be patient. We prioritise referrals to the team weekly and will see you as soon as possible from the time of referral.  

We regret that we may not be able see you if you arrive after you arranged appointment time. This is so that we can ensure adequate time to properly attend to other people attending the service with pre-arranged appointments. Thank you for your understanding.

How to make a comment, suggestion or share a concern

Receiving feedback from patients and carers enables the team to shape our services to their needs. If you would like to make a comment, suggestion or have a concern, then please speak with any member of the team who will be happy to receive your feedback. You may be asked to share a patient experience story using Care Opinion on discharge from the community diabetes specialist service .

If there is a matter that cannot be resolved then this will be referred to the diabetes team leads Doreen Kennedy/ Linsey Mawer , in the first instance and where necessary  to a senior member of staff in accordance with the NHS Greater Glasgow and Clyde Suggestions and Complaints Procedure.

Please note that in accordance with HSCP Policies, there is zero tolerance of verbal or physical abuse towards staff.
